I authorize Werner Coach to make inquiries regarding my driving record, employment history, criminal background, and any other investigation deemed necessary. I understand that employment is contingent upon satisfactory completion of all required background checks, drug testing, and medical certification. I authorize my previous employers to release information regarding my employment to Werner Coach. I understand and agree that my employment is "at will" and can be terminated by either party at any time for any reason.
Disclosure & Consent for Background Investigation
In connection with my application for employment with Werner Coach, I understand that a consumer report and/or investigative consumer report may be requested. This may include information regarding my character, general reputation, personal characteristics, mode of living, criminal history, motor vehicle record, and employment history. I hereby authorize Werner Coach and its designated agents to obtain such reports and to make any investigation of my background as may be necessary.
